Service description
Support for people with a learning disability who are over the age of 18, who are using behaviour that hurts themselves or other people. The staff/carers/social worker may be thinking that going into hospital or moving home may help. The outreach team will try and support people to stay in the community.
Referral criteria
Service is for adults with a learning disability and associated challenging behaviour(s).
