• Acute Recovery Team

  <p>The Acute Recovery Nursing Team specialise in delivering Electro-convulsive Therapy (ECT). We also run Clozapine blood clinics as well as Olanzapine depot clinic. These clinics operate during the working week and are staffed by a specialist nursing team.</p>

• Adult Community Mental Health Teams

  <p>The Adult Community Mental Health Teams (CMHT) are made up of consultant psychiatrists, psychiatric nurses, occupational therapists, social workers and psychologists providing a range of treatments, interventions and assistance to adults (people aged 16-65) suffering from difficulty with their mental health who have been referred by their GPs or other health professional.</p>

• Adult Eating Disorders Service

Following assessment you may be signposted to other services, offered individual or group therapy or inpatient treatment, depending on your needs. We work with you to achieve your goals in recovering from your eating disorder.</p> <p>Our service works in accordance with national guidance for the management of eating disorders set out by the National Institute for Health and Care Excellence (NICE).</p>

• Adult Speech and Language Therapy Service

  <p>Our Speech and Language Therapy Service for adults is provided across the community and hospitals of Leicester, Leicestershire and Rutland.</p>

• Arts in Mental Health

  <p>The arts in mental health team deliver a range of artistic projects for mental health service users whilst supporting service users in accessing artistic opportunities. We are dedicated to supporting service users in their artistic development and in the development of performances, exhibitions, recordings and DVD’s that promote positive images of mental health.</p>

• Assertive Outreach Service

  <p>Assertive Outreach (AO) is a model of care which has proved effective for people with a serious mental illness. The service aims to develop meaningful engagement with a diverse service user group in order to improve the quality of life of people with a history of severe persistent mental illness.</p>

• Care Navigation

  <p>Care Navigators within Leicestershire Partnership NHS Trust provide administrative support for Families, Young People and Children in Leicester, Leicestershire and Rutland.</p> <p>The team can provide help and support directly to parents and carers, health professionals and local partners such as schools, pre-schools, and local authority teams, when consent to share information is given.</p>

• Central Referral Hub/Unscheduled Care service

  <p>All referrals into Mental Health Services for Older People from across Leicester, Leicestershire and Rutland are received by the Central Referral Hub.</p>

• Child and Adolescent Mental Health Services (CAMHS)

• Children's Occupational Therapy Service

  <p>The service provides assessment, appropriate therapy, practical advice, strategies and equipment to ensure these children and young people are able to develop and achieve their academic, social and emotional potential.  Occupational therapists will work with children and families in the most appropriate setting, whether that is in clinic, home or at school.</p>

• Children's Phlebotomy Service

  <p>Our Children and Young Persons Community Phlebotomy Service provides a phlebotomy (blood sample taking) provision to all 0 to 16 year olds registered with a GP within Leicester, Leicestershire and Rutland (LLR), or under-18 if under the Community Paediatric service, SEND, a Looked After Child (LAC), or a CAMHS patient.</p> <p>It is committed to deliver a high quality phlebotomy service for children who require routine, urgent and fasting bloods.</p>

• Children's Physiotherapy Service

  <p>Our Children’s Community Physiotherapy Service is provided for children and young people with disorders or delays in their movement skills who will benefit from physiotherapy.</p>

• Children's Speech and Language Therapy Service

  <p>Our Children’s Speech and Language Therapy Service provide services to children and young people across Leicester, Leicestershire and Rutland.</p>

• Department of Clinical Neuropsychology

  <p>The Department of Clinical Neuropsychology is based at the Leicester General Hospital. We see both inpatients and outpatient who are having cognitive difficulties as a result of a neurological condition. These difficulties may be due to traumatic or acquired brain injuries, as well as chronic and progressive neurological conditions.</p> <p> </p> <p> </p>

• Community Integrated Neurological and Stroke Service (CINSS)

As an integrated multidisciplinary team we aim to empower our patients to manage their conditions and maximise their function.</p>

• Community Learning Disability Service

  <p>Our Specialist Learning Disability Services work in partnership with local communities to improve health and wellbeing and reduce health inequalities for adults with learning disabilities. This will enable adults with learning disabilities to lead more independent and fulfilling lives and will promote inclusion, choice and rights. The specialist learning disability service works with people who are 18 years old and upwards.</p>

• Community Paediatric Audiology Service

• Continence Service

• Criminal Justice and Liaison Diversion

  <p>The service assesses peoples mental health needs who have had any contact with the police or are involved in the criminal justice system whether they are a suspect or a witness.</p>

• Crisis Resolution and Home Treatment Team

  <p>The Crisis Resolution and Home Treatment Team provides a rapid assessment of people who are experiencing a mental health crisis of such severity that without the intervention of the team, a hospital admission would be necessary.</p>

• Diana Children's Community Service - Acute and Continuing Nursing Care

  <p>The service provides short term and continuing nursing care to children and young people up to the age of 18 years.</p>

• Diana Childrens Community Service

  <p>Our Diana Service provides care and support for children and families requiring special nursing care in a community setting.</p>

• Diana Childrens Community Service - Child and Family Support Service (CAFSS)

  <p>We help children, young people and their families to understand their thoughts and feelings about how they are being affected by illness. We do this by offering emotional support, using counselling techniques, and encouraging special therapeutic play that helps children and young people express themselves.</p>

• Diana Childrens Community Service - Macmillan Service

  <p>Our Children’s Macmillan Team provide nursing and support to children (0-18 years) and their families who are under the care of a paediatric oncology/haematology consultant. They provide support from diagnosis and throughout the course of the disease.</p>

• Diana Childrens Community Service - Cultural Link Worker

  <p>Our Diana cultural link worker helps with communication between health professionals and families who do not speak English or where English is their second language.</p>

• Diana Childrens Community Service - end of life care

  <p>Our Diana Service provides an on-call facility for children and young people at the end of life.</p>

• Diana Childrens Community Service - Leicester City Transitions Team for Health

  <p>Our Diana Leicester City Transitions Team for Health work with young people (aged 14 -19 years who live in Leicester City and have a City GP) who have ongoing healthcare needs. The team provide support to them and their families as they prepare for adulthood and the transition to adult healthcare services.</p>

• Diana Childrens Community Service - Paediatric Respiratory Physiotherapists

  <p>Our Diana paediatric respiratory physiotherapists assess and treat children with complex disabilities with additional respiratory issues in the community.</p>

• Diana Childrens Community Service - Short Break Care and Continuing Care Service

  <p>Our Diana Service offers families with children and young people with life limiting or life threatening conditions short breaks or continuing care.</p>

• Diana Childrens Community Service - Training

  <p>The Diana Children’s Community Service is a multidisciplinary team that provides care and support to families of children/young people with life-limiting/life threatening conditions. Within this service we provide training and assessments to healthcare and non-health professionals working with children/young people to carry out ‘health care tasks’.</p>

• Employment Support Service

  <p style="text-align: left;">The LPT <strong>Employment Support Service</strong> is delivered by Leicestershire Partnership NHS Trust; to adult patients open to community mental health teams,  psychosis interventi<img loading="lazy" class="wp-image-20966 alignright" src="https://www.leicspart.nhs.uk/wp-content/uploads/2020/07/IMG_2455-200x300.jpg" alt="" width="141" height="270" />on and early recovery (PIER) and assertive outreach.</p> <p>Its aim is to provide information, advice, guidance and support to find paid work, as part of an individualised recovery plan. • Enhanced Respiratory Assessment Service

  <p><strong>Enhanced Respiratory Assessment Service (ERAS)</strong> is an admission avoidance service providing specialist care by the Community Integrated Respiratory Service (CIRS) for patients experiencing exacerbations of their COPD (ECOPD). In these cases patients have a sustained worsening of symptoms beyond their usual day-to-day variations and need specialist support to stay at home.</p>

• Falls Prevention Service

  <p>The Falls Prevention Service aims to help patients maintain and improve their independence. Our specialist team works in partnership with local authority and voluntary organisations to provide targeted interventions to reduce the risk of further falls occurring.</p>

• Forensic community learning disability team

  <p>The specialist forensic community learning disability team (SFCLD team) provides health services for individuals with a mental disorder and/or learning disability (including neurodevelopmental disorders) who pose, or have posed, risks to others – and where that risk is usually related to their diagnosis.</p>

• Forensic Mental Health Services - Herschel Prins Centre

  <p>An inpatient and community service for people with a history of offending who also suffer from mental ill health.</p>

• Frail Older Person's Advice and Liaison Service (FOPALS)

• Healthy Together 0-19 (Health Visiting and School Nursing)

  <p>Healthy Together is a universal service for children and their families from 0-19 in Leicester city and 0-11 in Leicestershire and Rutland, and includes health visiting and school nursing services.</p>

• Heart Failure Service

  <p>Heart failure is the name given when you are diagnosed with a problem with the pumping action of your heart. Our Long Term Conditions Heart Failure Specialist Service comprises of heart failure specialist nurses.</p>

• Homeless Mental Health Service

  <p>A mental health service for people in Leicester who are homeless or do not have permanent accommodation/tenancy (eg. hostel, temporarily staying with friends/family).</p>

• Integrated Care Team

  <p>Our Integrated Care Team provide specialist mental health assessment and treatment within primary care settings (eg GP surgeries and health centres) for adults aged over 18 years who have a long-term condition which is linked to mental ill-health.</p>

• Integrated Community Specialist Palliative Care service

  <p>The Integrated Community Specialist Palliative Care service is a team of nurses and HCAs from LPT and LOROS. The team look after patients with life-limiting illnesses, including cancer, who have complex palliative care needs, especially pain and symptom management, as well as patients who are in the last days of life. This can include when appropriate intervention has failed to control symptoms; when symptoms are escalating and there is incomplete symptom control; when patients or families/carers require psychological and spiritual care or if there are any other problems that other healthcare professionals are unable to manage.</p> <p>The team offers different levels of support to patients, families/carers, including; telephone advice, one-off assessments, on-going management and personal care.</p> <p>Working in partnership with other professionals, including community nurses, GPs, palliative care medical teams, and social services. They offer advice and support to other members of the community health service about their patients.</p>

• Learning disabilities outreach

  <p>Support for people with a learning disability who are over the age of 18, who are using behaviour that hurts themselves or other people. The staff/carers/social worker may be thinking that going into hospital or moving home may help. The outreach team will try and support people to stay in the community.</p>

• Learning Disability Primary Care Liaison Nurses

  <p>We work together with Primary Care to improve access to health care and reduce health inequalities for people with a learning disability.</p>

• Learning Disability Short Breaks Service

  <p>Our Short Breaks Service provides short breaks for adults with learning disabilities and associated physical and sensory disabilities, challenging behaviours or autism.</p>

• Leicestershire Psycho-oncology Service (LPOS)

  <p>The Leicestershire Psycho-oncology Service provides psychological and psychiatric care to patients who have a current or recent diagnosis of cancer that is impacting upon their psychological wellbeing.</p>

• Leicestershire Recovery College

  <p>Leicestershire Recovery College is an NHS college that provides a range of educational courses and resources for people with lived experience of mental health challenges, their friends, and family and Trust staff.</p> <p>The college uses a recovery based approach to help people recognise and develop their personal resourcefulness in order to become experts in their own self-care, make informed choices about the assistance they need to do this, and do the things they want to do in life.</p>

• Looked After Children's (LAC) Team

  <p>The Looked After Children’s Health Team (LAC Team) aim to promote the health  and well-being of children and young people who are looked after, including unaccompanied asylum seeking children.</p> <p>An Initial Health Assessment for all children and young people will be undertaken by a Paediatrician, following that, children and young people age 5-18 years will have  a Looked After Children’s Nurse and children under 5 years will be cared for by their Public Health Nurse- Health Visitor.</p> <p>The team supports the child/young person and their carers throughout their time in care with health assessments, packages of care, advice, attending review meetings and making referrals to other services when required.</p>

• Maternal Mental Health Service

  <p>We are a psychology-led, trauma-informed service. We have a small multi-disciplinary team which includes a psychology team, a specialist mental health midwife and a specialist mental health nurse.</p>

• Medical Psychology

  <p>The Medical Psychology Department is a service for adults who are having difficulties with managing the impact of medical/physical health problems on their psychological well-being, or are finding that their mental health is having a direct impact on their physical health.</p>

• Mental Health Central Access Point

  <p>Anyone needing mental health support for themselves or others can call this service on <strong>0808 800 3302</strong>, 24 hours a day, seven days a week.</p>

• Adult Liaison Psychiatry Service

  <p>Our Liaison Psychiatry Service provides assessment and treatment for people who experience mental health problems as a result of physical illness.</p>

• Mental Health Services for Older People (MHSOP) Community Mental Health Teams

  <p>Our MHSOP Community Mental Health Teams provide more routine (non-urgent/non intensive) multi-disciplinary assessment and intervention for patients of any age with a complex presentation of dementia or those over 65 with a complex functional illness. This intervention takes place in a community setting, for example, patient’s own home.  The team comprises of Psychiatrists, Clinical Psychologists, Mental Health Nurses, Occupational Therapists and Health Care Support Workers.</p>

• Mental Health Services for Older People (MHSOP) In-reach Team

  <p>Our MHSOP In-reach Team provide assessment and specialist support to patients with dementia who are living in a care home.</p>

• Mental Health Services for Older People (MHSOP) Inpatients

  <p>Our inpatient units look after people of all ages with dementia and adults over the age of 65 who have decreased mental function.</p>

• Mental Health Services for Older People (MHSOP) Memory Service

  <p>Our MHSOP Memory Service provides diagnostic assessment and treatment for people with dementia within a clinic setting, care home or their own home.</p>

• Mental Health Services for Older People (MHSOP) Outpatient Service

  <p>Our MHSOP Outpatient Service provides assessment and treatment for people over 65 years with moderate/complex functional mental health issues that require medium to long-term care within a clinic environment.</p>

• Mental Health Support Teams in Schools (MHST)

  <div class="wrap | body__wrap" role="document"> <div class="content | body__content"> <p>Mental Health Support Teams in Schools (MHST) are part of a national programme to reduce inequalities and increase access to mental health support for children and young people.</p> <p> </p> </div> </div>

• Mental health urgent care hub

  <p>The mental health urgent care hub is based at the Bradgate Unit, on the site of Glenfield Hospital, and is staffed by mental health practitioners with the expertise to treat people of all ages; this includes mental health nurses, support workers, and consultants. It is specifically for people with mental health needs that don’t need any physical health support from an emergency department.</p>

• Mental Health Wellbeing and Recovery Support Service (MHWRSS)

  <p>This service is open to anyone aged 18+ who is experiencing emotional and mental health problems, as well as their carers. Its goal is to help you to maintain and improve your mental wellbeing, become empowered to achieve your goals, and to live a meaningful life. You don’t need a mental health diagnosis to use this service – it is open to people who would benefit from support offered. Please choose the organisation that is local to you.</p>

• Mill Lodge, Huntington's Disease Inpatient and Community Service

  <p>Mill Lodge is an inpatient unit for people with Huntington’s disease. The community service cares for people who have been diagnosed or are at risk of developing Huntington’s disease together with families and carers of people affected.</p>

• Musculoskeletal (MSK) Therapy

  <p>Our MSK Physiotherapy Service provides outpatient clinics in community hospitals and health centres across Leicester, Leicestershire and Rutland and aims to help those who are affected by a musculoskeletal disorder of an acute or chronic nature.</p>

• Neighbourhood Mental Health Cafes

  <p>The Neighbourhood Mental Health cafes are part of Leicestershire Partnership NHS Trust’s local support for people who need immediate help with their mental health.</p>

• Nutrition and Dietetics

  <p>Our Nutrition and Dietetic service provides advice for adults and children regarding their nutrition, and also provides training and education for other health professionals to enable them to promote good nutrition.</p>

• Op Community

  <p>Op Community is a telephone line for the armed forces community (including veterans, reservists, serving personnel, families and the wider armed forces community) to offer support and guidance around navigating NHS services and advice regarding other services that can support with issues.</p>

• Outreach Team for Adult Learning Disabilities Service

  <p>Our Outreach Team work with individuals who have a learning disability who require support due to the presentation of severe challenging behaviour and who, because of the risks associated with those behaviours, pose a risk to themselves and/or those who care for them.</p>

• Paediatric Psychology Service

  <p>Our Paediatric Psychology Service offers psychological assessment and intervention to young people up to their 19th birthday who are experiencing psychological difficulties as a result of a chronic physical health condition.</p> <p> </p>

• Perinatal Mental Health Service

  <p>The Perinatal Mental Health Service is a team of health professionals that provide personalised and specialist care to people living in Leicester, Leicestershire and Rutland who have moderate to severe mental health problems relating to pregnancy, childbirth and the first year following a child’s birth (also known as the perinatal period).</p>

• Podiatry

  <p>Our Podiatry Service aims to keep the population mobile, help users keep their independence and maintain their quality of life by providing an expert and complete range of foot and lower limb care, for those with a clinical need.</p>

• PAUSE/At Risk Mental State (ARMS)

  <p>This service will launch in Autumn 2023, initially in a targeted geographical area rather than across Leicester, Leicestershire and Rutland. • Psychosis Intervention & Early Recovery (PIER) Team

  <p>The aim of our service is to help people (aged 14 to 64 years) who have experienced a first episode of psychosis to recover, to manage any ongoing difficulties, and to minimise the chance of potential relapse. We also offer support to families.</p>

• Pulmonary Rehabilitation Service

  <p>Our Pulmonary Rehabilitation Team are part of the Long Term Conditions Team and deliver pulmonary rehabilitation to patients in a wide variety of community venues.</p>

  <p>The service:</p> <ul> <li>Offers a care after custody service that seeks to improve the continuity of care of vulnerable people leaving prisons/Immigration Removal Centres (IRC)</li> <li>Supports service users’ transition to community-based services and safeguards health gains made whilst in prison/IRC</li> <li>Offer liaison, advocacy, signposting, and engagement with community-based health and support services</li> <li>Improves service users’ wellbeing, reduce inequalities and address health-related drivers of offending behaviours.</li> </ul>

• Respiratory Service

• School Aged Immunisation Service

  <p>The School Aged Immunisation Service supports and delivers the School Aged Immunisation Programme across Leicester, Leicestershire and Rutland. The service provides immunisations for children and young people who are in school or home educated. Vaccinations are provided either in schools or through community clinics and in some cases, through home visits.</p> <p>The service is provided and supported by a team of skilled and experienced staff. Our immunisation nurses have experience in all aspects of immunisation and vaccination.</p> <h3>The service offers the following immunisation programmes:</h3> <ul> <li>Seasonal flu vaccination to protect healthy children against flu (in school years Reception – year 11)</li> <li>Seasonal flu vaccination to protect children in special schools against flu</li> <li>Human Papillomavirus (HPV) vaccination to protect young people against various forms of cancer (offered from year 8)</li> <li>3-in-1 teenage booster and Meningitis ACWY vaccinations to protect young people against tetanus, diphtheria and polio and meningitis strains A, C, W and Y (offered from year 9)</li> <li>Measles, Mumps and Rubella (MMR) vaccination for under immunised young people (offered from year 8 and by invitation only)</li> </ul>

• Rehabilitation Services - Stewart House and The Willows

  <p>Our Mental Health Rehabilitation Services are provided from two inpatient units, Stewart House and The Willows. These are inpatient units providing care for people with severe and enduring mental illness.</p>

• Specialist Autism Team

  <p>The Specialist Autism Team (SAT) is a multi-disciplinary team committed to improving the quality and care for people who are autistic. The team provide post-diagnostic support to autistic people aged 14 and upwards, including adults.</p>

• The Agnes Inpatient Unit

  <p>The Agnes Unit is an inpatient service for adults (aged 18 years and over) with learning disabilities. The unit provides an inpatient service for individuals whose mental health, behaviour and risk cannot be supported in the community. The unit has 12 beds in pods with all bedrooms having en-suite facilities.</p>

• The Bradgate Mental Health Unit

  <p>An acute mental health admissions unit with seven acute admission wards and a ten-bedded male Psychiatric Intensive Care Unit (PICU).</p>

• The Involvement Centre and Café

  <p>The Involvement Centre is an information, IT and social resource, open to inpatients, outpatients, service users, carers and visitors.</p>

• The Mett Centre

  <p>The Mett Centre is a mental health day resource centre in Leicester city centre, offering recovery-focused support, through individualised programmes of meaningful activities, physical and mental health promotion, social inclusion and therapeutic interventions. Our service is available to adults with complex and enduring mental health difficulties who have a Consultant Psychiatrist at Leicestershire Partnership NHS Trust and may be particularly suitable for individuals who are isolated socially and may currently find it difficult accessing mainstream services.</p> <p> </p> <p> </p>

• Therapy Services for People with Personality Disorder (TSPPD)

  <p>Therapy Services for People with Personality Disorder provides the opportunity to address the ways in which traumatic experience has shaped someone’s emotional development, impaired their mental health and damaged their capacity for thinking, trusting and relating.</p>
